Request for response: K-5 food service request form

appleThe U.S. Department of Agriculture (USDA)  is offering breakfast and lunch on every day of instruction, free-of-charge, to all children from birth to age 18, through Dec. 31 or until USDA funding for the program is exhausted.

K-5 Schools Food Request

In-Person Instruction Students:
If your child(ren) are attending in-person instruction and you would like them to participate in this program, please complete this form for EACH child that is attending in-person instruction. Once you have completed the form your child(ren) will be provided meals in school on days they attend and meals will be sent home for the days they aren’t in school as follows:

Students A-K (Green Days) will bring home two days’ worth of meals on Mondays and one-day worth of meals on Thursdays.

Students L-Z (White Days) will bring home two days’ worth of meals on Tuesdays and one-day worth of meals on Fridays.

Remote Instruction Only Students or Preschool Aged Children:
If your child(ren) are strictly remote learning or are preschool aged, do not need to fill out this form. Please feel free to stop by either of our Grab and Go locations at Otisville Elementary or Minisink Middle school from 8 to 10:30 a.m. days when instruction is being provided.
