We’re pleased to announce an update to our COVID-19 Return to School Policy, which previously required a doctor’s note, effective immediately.

The new policy, approved by Dr. Sara Little, the district’s medical director, allows for the use of this form to clear your child after being sent home by the school to quarantine due to exposure in school. It can only used for this purpose.

NOTE: This form may not be used if your child is exposed outside of school or if your child has travelled outside of New York.  You  must continue to follow current New York State Department of Health (NYSDOH) and Orange County Department of Health (OCDOH) guidelines.

REMINDER: If your child has been quarantined following exposure to COVID-19 in the school setting, this form may be used to obtain clearance from a school nurse instead of having to get clearance from your healthcare provider.
